Match Report Directions:

 This report form is to be used for the First Side matches of all Ohio League, Tournament and Referee requested matches. It is the home club’s responsibility to turn the completed form (both home and visiting sides rosters
listed) over to the referee before kick-off.

Home Club
1. Complete the Home Club section of the form in full. Place a "X" in the Front Row Column for all of the players on your team that are capable of playing in the front row.
2. Give the form to the Visiting Club and have them complete the Visiting Club section of the form in full. Place a "X" in the Front Row Column for all of the players on your team that are capable of playing in the front row.
3. Officials from both clubs verify Identification and CIPP enrollment for all participating players.  They then sign the form.
4. Return completed form with a stamped envelope addressed to:

Referee
1. Confirm with the club officials that the CIPP & insurance were provided.
2. Complete the Referee section of the form and Additional Comments.
3. Mail in the form on the day of the match.
